InMarket COVID-19 Client/Partner Event Liability Release Form  

Due to the ongoing COVID-19 pandemic, InMarket requires all clients, partners, and guests attending InMarket-sponsored events to complete this release form: 

I agree to the following: 

• I do not, and everyone in my household does not, currently have COVID-19. No one in my household has experienced the symptoms of COVID-19 within the 14 days prior to the event. 
• I have not, and everyone in my household has not, been diagnosed with or tested positive for COVID-19 within the 14 days prior to the event.
• To our knowledge, I have not, and everyone in my household has not, been exposed to anyone with COVID-19 within the 14 days prior to the event. 
• InMarket and its employees will not be liable for any exposure to COVID-19 or any other contagion I may experience by attending the event. 

By submitting or attending the event, I agree to each above statement and I voluntarily release InMarket and its employees from any and all liability for any possible exposure or harm due to COVID-19 or any other communicable diseases. I knowingly and voluntarily accept the risk of participating in the event.
